Office Visits and Fees

ON YOUR FIRST VISIT: You will receive my online intake form after you make your first appointment but if you don’t, please let me know and I’ll re-send it. You may also choose to arrive 5-10 minutes early in order to fill out my health history form before your session or click on the link to download it and bring it with you. I also ask that you bring a list of any current medications as well as the results of any imaging relating to your condition, if you have them. Please keep in mind that we will need at least a few minutes of your first appointment to go over your health history, complete any additional forms and do a brief assessment before starting treatment.
LOCATION: My address is 246 GARDEN AVENUE, BACK DOOR. The closest subway station is Dundas West and the 504 streetcar stops just steps from my door. There are signs outside my location to help direct you but if you get lost, please call or text at 416-560-3469 and I’ll help you find me.

PARKING: There’s 1 hour street parking right outside between 10am and 6pm. For appointments that begin earlier than 10am, please DO NOT PARK ON GARDEN OR YOU WILL GET A TICKET! Another option is to park on Fern Avenue (1 block north of Garden) from 8am. Otherwise, please park on Parkside or use the pay parking on Roncesvalles.

PAYMENTS: Prepayment is optional using a credit card at time of booking using the online system. Onsite, I accept cash, cheque, Debit, etransfer, Visa, MasterCard and AmEx. An insurance receipt is always provided.
BOOKING: You may contact me via phone, text or email in order to book your appointment. To book online, click here or use the “Book Now” button in the top menu.
CANCELLATIONS/REFUNDS: Please provide 24 hours notice for any changes or cancellations or the full fee for that visit will be charged. A phone call or text will reach me faster than email if that does happen. Once you’re a client, you can directly access your account by logging into my booking software here.
A prepaid service that is cancelled prior to receiving it qualifies for a refund. A service fee may apply. Alternatively, a balance can be applied to your account and used for future sessions.
GIFT CERTIFICATES: To purchase online, click here. Available options are listed under “In Office Service Gift Certificates“. Gift certificates may be purchased in person, by phone or by sending a cheque to my clinic address (246 Garden Avenue, Toronto ON, M6R1J3). Alternatively, a payment may be made via etransfer to firstname.lastname@example.org. Once payment has cleared, the certificate can be sent to you or directly to the recipient. All gift certificates are valid for 1 year from the date of purchase.
